Suitability Form Update NWL has revised the Annuity Suitability Questionnaire, form # DM-1174.Rev.2.13 with form # DM-1174.Rev.1.15.

The previous version of the Annuity Suitability Questionnaire will not be accepted after June 25th, 2015.
As a result of the updated form, the Deferred Annuity Certification Form DM-1243 will no longer be required in states that require the Suitability Questionnaire DM-1174.Rev.1.15.
